Step One - Health Appraisal Questionnaire

As part of a comprehensive program, the VitaMax^TM Nutritional Counseling Program analyzes your individual needs through a confidential patient evaluation form. The patient questionnaire inquires about your current health, your health history, as well as symptoms you may be experiencing. Your answers help our specially trained pharmacist to target specific vitamin, mineral and nutritional needs specific to your body. Step Two - Hormone Levels

Test results from lab work may reveal the effectiveness of your current vitamin and mineral supplementation program and your unique body requirements.

It's important that you do not stop your current regime. The lab work will uncover:

Ö what is duplicated

Ö what isn't needed.

The tests can be purchased directly from Healthway Compounding Pharmacy, taken in the privacy of your home, and mailed into the laboratory for analysis.

Step Three - The Consult

Your personal consult can be scheduled in person or by phone. During your scheduled consultation with one of our highly trained pharmacists, your patient questionnaire and lab work will be reviewed, as well as any other concerns you may have. In addition, specific nutritional interventions may be addressed at this time.

You have our personal guarantee that your VitaMax^TM Nutritional Counseling Program is individually tailored to meet your body's specific needs.

Consultation
$125.00 for up to one hour

Step Four - Follow-up

During this second consultation our pharmacist will review your progress and make additional recommendations as needed.

Insurance

Healthway Compounding Pharmacy does not bill insurances for consultations or prescriptions. However, we will be happy to assist you with the proper paper work in seeking reimbursement for your compounded prescription(s).
